How does Patient Navigation Work?
  • Bridges cultural and linguistic barriers or gaps
  • Finds Financial Resources
  • Tracks Appointments
  • Coordinates Transportation
  • Refers to Appropriate Services (e.g. Social Services)
  • Ensures that Patients do not "fall through the cracks"

What does a Patient Navigator do?

A Patient Navigator is a type of case manager who guides the patient, family members and caregivers through the complex and often frustrating barriers of the healthcare system to coordinate services with social services, health care providers, financial clearance and community social services to enable the patient to access available resources thus increasing patient satisfaction.
